severe and persistent mental health issues that may prevent them from maintaining independent
functioning without additional assistance on a short-term or long-term basis. TCN has a

TCN provides individual assessment to determine a plan that meets your specific need for
co-ordination of services. Your case manager will assist you in working towards
your goals and meeting your basic needs.
TCN Community Support Service clients may utilize the resources available through
individual and group mental health and
drug and alcohol counseling services.
TCN's Adult Psychiatric Services is led by Chief Medical Officer and Board Certified
Psychiatrist, Dr. Franklin Halley. He is supported by a staff of adult auxiliary
Psychiatrists, two Psychiatric Advanced Practice Nurses, and three Registered Nurses.
Clients of Adult Psychiatric Services will receive compassionate diagnostic,
prescriptive, and ongoing care.
During your initial Psychiatric visit, you will work with your nurse to determine
what eligibility requirements you meet to match you with various state and benevolent
programs which may assist with the cost of your medication. During your participation
in Community Support Services, your case manager will assist you with
maintaining your medication schedule.
Community Support Services works closely with this dynamic day treatment program which
enables clients to reduce their need for hospitalization and increase their ability to
manage symptoms of their mental illness. This program also provides a vital adjustment
period to those recently released from the hospital. Transportation is available
to day treatment.
TCN recognizes that many clients have both mental illness and chemical dependency
treatment needs. This highly specialized treatment option addresses the challenges
of working toward recovery when dealing with multiple issues.
Family members may work individually with a Community Support Specialist to learn
the fundamentals of mental illness and how they can promote recovery.

TCN provides a 24 hour crisis hotline for clients and non-clients of our agency.
In the event that you experience a crisis situation including feeling suicidal
or homicidal you can call the hotline 24 hours a day, every day of the year. You
will be connected with an experienced hotline operator and provided individualized
attention to your needs and developing a plan to meet your goals.
Community Support Services offers financial management assistance and education
to work towards the goal of building your ability to make wise spending and
budgeting decisions.
Your case manager will address your individual housing needs and match you with
appropriate available resources always taking great care to maintain your highest
level of independent living possible.
TCN links you to daily recreational and social centers to keep you actively
involved with people and activities that will promote your recovery goals.
Your case manager will work with you to address transportation needs. You may
qualify for van transportation to treatment services.
Community Support Services works with BVR, Greeneworks, and CDS to assist
you in meeting your educational and employment goals.
