The answers to these commonly asked questions may help you determine if you or someone you care for may benefit from Community Support Services (CSS).

Q: What is case management?
A: Case management is intended for persons whose mental health needs require active assistance and support to function independently in the community. Services may be short-term, just a few contacts for a specific purpose, or long-term for ongoing support and monitoring.

Q: How does case management work?
A: A TCN Behavioral Health Services staff member (Case Manager) can be assigned to assist you with a number of activities related to specific needs that you have. The case manager will spend time with you, helping you to identify these needs and support you in meeting these needs by linking, advocating, supporting, and coordinating services.

Q: Who would benefit from case management?
A: Case Management will help individuals who are impaired by their mental illness and needing assistance with:
  • Symptom Management/Improving Daily Living Skills
  • Medicaid or Disability benefits
  • Food Stamps
  • An advocate to help them access services of other agencies
  • Housing
Q: How can case management help me improve my daily functioning and work towards independent living?
A: A case manager can help you to accomplish a variety of things. Some examples are:
  • Assessment of your needs and help with obtaining Medicaid or disability assistance, food stamps, safe housing, or referrals to vocational services.
  • Assistance in achieving personal independence in managing basic needs and development of daily living skills if needed, including money management.
  • Coordination of your treatment plan if you have more than one provider, or are involved with more than one agency.
  • Linkage and advocacy with other systems such as Greene Metropolitan Housing Authority, Children's Services Bureau, and the Ohio Department of Job and Family services.
  • Symptom monitoring, for frequent contact during crisis situations, or following a hospitalization.
  • Assisting you and your family or significant others with education about your illness and assisting you with the recovery process.
Q: How much does this service cost?
A: Medicaid pays for case management services. If you have no income (at 0% pay) the Eastern Miami Valley Board will pay for the services. At your initial appointment a financial assessment will be conducted to determine your assistance eligibility.

Q: How do I get Community Support Services?
A: If you are not a current TCN client, you can call 937-376-8700 and request an intake appointment. During your initial appointment your counselor will assess needs in all areas and if appropriate make a referral to case management. If you are already receiving services at TCN, discuss your interest in Community Support Services with your counselor.
